We have a number of forms for you to download from this website. You can download in one of two different formats; as a Word document or as a PDF file.
If you choose a Word document, you will be given these choices:
- Open the file in the appropriate program. If you use this routine, we suggest that you also be sure to save the form to your computer.
- Save the file to disk. If you use this routine, you will need to remember where you have saved it and then open it later.
If you choose the PDF format, you will need to have a PDF reader in your computer (see bottom of page for further details). The form will appear as a PDF file and you can click on the Save File button to save it to your computer.
In either case you will be able to print out the form so you can fill it out and send it back to us.

Donation form
The Donation Form can be used when making a gift or loan and includes a Standing Order Form should it be required. This form must be filled out by hand, signed and returned to the address given on the form. Pleased click on the file format you would like to download to your own computer.
MIFS (Maintenance & Improvement Fund Supporters club) form
